Free care navigation services are the focal point in Talem’s care philosophy. As a part of our commitment to being an active member of the communities we serve, at Talem Home Care, we want to become a trusted resource and information center for all to turn to when they don’t know where to get answers.

To us, care navigation is defined as a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive needs through communication and available resources to promote quality, cost-effective outcomes.

Navigating through the complexities and the confusion of today’s healthcare system can cause you or your loved one to experience failures in coordination and education, particularly at points of transition. A transition can be a change of physical location (ex: Hospital to home or rehab to home), or it can be a change in your overall health. Our goal is to assist you through these times, so all you have to focus on is getting better.

These care navigation services may include but are not limited to:

  • Helping families who may live out of town take care of their loved ones. Having us be your eyes and ears;
  • Working in partnership with you, and if applicable, your responsible party, to understand your care needs and wants so that we can advocate on your behalf;
  • Offering guidance and recommendations should you require additional support from a third-party health care provider, such as rehab, therapy, home health, hospice, mobile pet care, mobile podiatry, etc.;
  • Coordinating care with your hospital or rehabilitation center to ensure a smooth transition back to your home;
  • Routinely communicating with physicians, discharge planners, social workers, and other health care professionals regarding your condition’
  • Benefits checking to help you understand what your insurance covers; and
  • Assisting you to fill out the application for the Veterans Administration Aid & Attendance program.n

You do not need to be a client to get our help.

We understand there may be times when you or a loved one might end up in the hospital, or a doctor informs you that you might face some difficult health issues. You might need help finding medical equipment, or you might need a referral for home health, hospice, house call doctor, mobile pet groomer, or any other service that can help us as we age. Our mission is to help you find the answers you need and to get the right quality services.

What is Included in Care Navigation Services?

A skilled Care navigator acts as a central coordinator, addressing the social, emotional, physical, and healthcare needs of your loved one. It’s important to note that not all care managers specialize in every area, so finding someone with experience in your specific needs is crucial. Here are some of the ways in which Care Managers provide assistance:

  • Conducting a thorough in-person assessment: This assessment enables the care manager to understand your unique needs and customize plans accordingly. Through comprehensive interviews covering various relevant aspects such as everyday activities, nutritional status, safety, memory, depression, finances, insurance, and health history, the care manager gains insight into your loved one’s health and living situation.
  • Creating a personalized care plan: Based on the assessment results, the care manager formulates a care plan that includes recommendations and referrals for local community options. The care manager takes the time to explain the details of the plan, the reasoning behind the recommendations and helps prioritize the steps to be taken.
  • Arranging tailored services: The care plan serves as a guide for organizing services based on the identified needs. Care Managers can assist with crisis intervention, counseling and support, consumer education and advocacy, money management, referrals, decisions regarding suitable housing options, assistance with relocation, advice on power of attorney or guardianship matters, acting as a liaison for distant families, ensuring the well-being of your loved one, and alerting you to any emerging issues.
  • Monitoring ongoing needs: Once services and arrangements are in place, regular monitoring becomes vital. Frequent re-evaluations help in making necessary adjustments. The Care Manager keeps track of your loved one’s capabilities and functions over time, ultimately enhancing their quality of life and providing peace of mind for you.
